In daily hospital settings, patients are often submitted to surgical and / or invasive procedures without information on the risks and possible impacts on quality of life. Objective was to evaluate the level of participation and understanding of patient and responsible regarding the process of obtaining the Informed Consent Term. Methodology consisted of descriptive and exploratory with quantitative approach. Data collection was performed during the first half of 2016, the questionnaire was applied to all patients who met the eligibility criteria and were hospitalized in the medical, surgical and maternity clinics. As a Result, it can be seen that this situation is not experienced in many cases in the hospital routine, since among the assertions presented to the patients it was found that in almost all the questions the modal value one concerning the totally disagree option was the one that prevailed the most, the lack of informed consent by the respondents and therefore also disagreed as to the fact that they received counseling before signing the informed consent form. Conclusion The informed consent term is an important document attached to the patient's chart and serves as a support for health professionals for invasive intervention, and that such document is also fundamental for the moment when the patient or caregiver is advised of the risks and potential adverse events associated with the procedure.
