A pneumonia necrotizante é uma complicação rara da forma comunitária com uma fisiopatologia ainda mal definida. Apesar de ser uma condição rara e grave, os pacientes pediátricos tendem a responder bem à pneumonia necrotizante, muitas vezes obtendo remissão do quadro apenas com terapia clínica. Na faixa etária pediátrica a epidemiologia da pneumonia necrotizante está vinculada a casos que acometem crianças pequenas, mesmo que previamente saudáveis e sem os fatores de risco associados 1,2O objetivo desse artigo é relatar um caso de pneumonia comunitária associada a derrame pleural e broncoespasmo em pré-escolar que teve evolução desfavorável para pneumonia necrotizante e correlacionar com pesquisas nas bases de dados do Scielo e pubMed.

Necrotizing pneumonia is a little-talked about complication in the community with a pathophysiology still poorly
defined. Despite being a rare and serious condition, pediatric patients tend to respond well to necrotizing pneumonia,
often obtaining remission of the condition only with clinical therapy. In the pediatric age group, the epidemiology of
necrotizing pneumonia is linked to cases that affect young children, even if previously healthy and without the associated
risk factors. The present case report is about a preschooler with a history of febrile peaks, tachypnea, prostration, changes
in pulmonary auscultation and edema in the upper limbs and face, being diagnosed with community-acquired pneumonia
associated with pleural effusion and bronchospasm, which evolved unfavorable for necrotizing pneumonia. The current
study aimed to report the history of this patient and correlate with cases described in the literature. The information
contained in this work was obtained through a retrospective analysis of the patient’s medical record and correlated with
case reports and reviews present in the literature so far, searched in Portuguese, English and French in the PubMed,
Scielo databases and Google Scholar, from 2010 to 2017. It is observed that necrotizing pneumonia is a rare complication
of community-acquired pneumonia and that it needs an early diagnosis to avoid unfavorable outcomes and surgical
interventions.
